Back to lactation - until a pregnancy is near completion and a newborn has suckled every few hours for a few days - the breasts are not fully developed into milk making mode. After that first pregnancy, than the milk and know-how return readily for next babies (who can have their own quirky feeding issues to get through). Puberty blockers in males may make the penis not grow, puberty blockers in females may make the mammary glands not fully develop. Some women naturally (no puberty blockers) may have both or one breast that is at an underdeveloped stage and will not be able to make milk well even with an infant suckling. The underdeveloped breast is likely to look smaller, more adolescent early development.

Hormones in transwomen that lead to their mammary glands developing more - I don’t know if milk would be achieved with suckling stimulation but it might.

For a woman with a premature baby who is too weak to suckle, a double electric pump and pumping around the clock every two hours replicates what a newborn would do and can establish a milk supply for the premature infant and a little supplemental fortifier nutrients might be mixed into the pumped milk.

Breast milk is high tech in a natural sense - it is modified for different stages of infant development. Newborn colostrum is lower in fluid, richer in fats and calories and has other stimulating growth factors. Regular newborn milk arrives with a painfully extended chest and a need for that baby to suckle. Hand expressing a little might be needed to soften the overfull breast enough for the baby to be able to latch on. Early milk is not as creamy rich as colostrum but it is more nutrient dense than milk being produced for an older baby. And milk for a toddler might be even thinner and maybe less sweet - naturally promoting weaning to other foods.

The fat content of the person’s diet will be directly reflected in the types of fats in the breast milk. Omega 3’s and good fats, or fake homogenized trans-fats and polyunsaturated oils, the ratio in the diet will be similar in the milk being produced. For some other nutrients, if the diet is kind of malnourished the body will compensate somewhat and take from the mother’s body stores - depleting her of trace minerals like calcium and magnesium.

A negative of modern life - toxins in the mother’s body are filtered by the placenta and the uterus has less than her average load. The breasts don’t filter it out though, instead almost seem to concentrate it so the baby is getting a bigger load than her average body load. That electric double pump, a lot of work, but might be a chemical detox strategy - induce milk production and pump and dump for a while before trying to get pregnant and then the baby may be exposed to less of the accumulated body burden of toxins that the mother had built up from her fetal to present life.

Oligofructosaccharides in breast milk would be indigestible for the human infant but essential fiber food for a healthy microbiome. Breast milk has it, which indicates that a healthy microbiome is part of a healthy human infant - we are symbionts and part of nurturing is taking care of the baby's gut microbes. Natural vaginally delivery has been found to be healthier as it is starting the infant off in life with microbes picked up from the mother's vaginal canal.

Formula is now being made with oligofructosaccharides. It seems to help babies on formula. We study nature to try to replicate it for those in need of help.

Man tries to replicate nature with incubated babies but it is never likely to be as successful as nature's design. The other concern about a move towards incubated babies is lack of a personal protector.
